To become a member of Optical Women's Association, please complete the on-line application form below. Upon receipt of your application, we will invoice you in the amount of $125 US. If submitted by February 1st or if more than one person from your organization is a member of Optical Women's Association, membership fee will be invoiced at the rate of $95 US per member.
